We are located in the heart of Manchester Village, Vermont. Our updated facility offers a bright, clean, and comfortable environment with private treatment rooms and technologically advanced equipment. You will receive high-quality, one-on-one care only from a licensed physical therapist during each visit.

FAQ

What insurance plans do you accept?

  • We are participating providers with the insurance carriers listed below. If your carrier is not shown below, please contact your insurance company to find out if you have out-of-network Physical Therapy benefits.
  • It is your responsibility to know your full Physical Therapy benefits (i.e., Copay, Coinsurance, Deductible, Number of Visits Allowed) before your Initial Evaluation appointment. It is also your responsibility to know if pre-authorization for Physical Therapy is required. You will be responsible for any payments not covered by insurance.

What should I bring to my first appointment?

  • Photo ID
  • Insurance card(s)
  • Prescription/referral from your physician
  • Any diagnostic imaging or medical reports you may have
  • Credit card, as per our Payment Agreement
  • Payment (cash, check, or credit card) for copays or self-pays, if applicable
  • Reading glasses, if needed
  • Assistive devices (i.e., crutches, cane, walker, etc.), if used
  • Any brace you may be using

What should I wear?

  • Loose-fitting clothing you can easily move and stretch in
  • Sneakers or supportive shoes (no sandals or flip-flops)
  • Shorts, if you have a lower extremity injury or issue
  • Sleeveless shirt, if you have an upper extremity injury or issue

How long will my appointment be?

  • Allow one hour for your Initial Evaluation appointment (Please arrive 15 minutes prior to your appointment time to fill out paperwork.)
  • All follow-up visits are 45 minutes.
  • Please call if you are running late for any reason. If you arrive more than 15 minutes after your appointment time, we will not be able to see you, and you will be charged the $50 no-show fee.

Do I need a referral?

  • A referral is required for Medicare and Vermont Medicaid patients.
  • For other insurance carriers, we recommend you obtain a referral from your primary care physician or orthopedist, even though it may not be specifically required.

What is your cancellation policy?

  • We require 24-hour notice for all cancellations.
  • Missed appointments or cancellations made within 24 hours of your appointment time will incur a $50.00 fee.
  • If you have three no-shows, we reserve the right to discontinue your treatment.
